Your role at St John of God Social Outreach We are seeking applications for a Clinical Nurse (Mental Health) to join our Medicare Mental Health Service on a permanent full-time basis, in Armadale, WA. Our team provides a safe and welcoming spaces for locals to walk in and access free mental health support and services - no referral, Medicare card or appointment needed. Staffed by qualified mental health professionals and people with lived experience, the centres provide immediate support to adults in distress, including their families, friends and carers. We can also connect people with other support services, such as employment assistance, to support them in the future. You will provide a high standard of consumer care, acts as a clinical resource and effectively manages resources to deliver quality consumer outcomes in the relevant Medicare Mental Health service setting. The Position Applies advanced clinical knowledge and skills in the coordination of direct care to consumers. Undertakes comprehensive Mental Health Assessments and develops care plans in collaboration with the multidisciplinary teamProvision of mental health support to consumers including clinical risk assessment and safety planning, brief intervention counselling, referrals to other services, and co-facilitating psychoeducational group sessions. Acts a clinical resource, maintains and develops clinical skills relevant to area of practice. Initiates reviews and coordinates care of consumers with complex health problems. Serves as a consumer advocate, protecting the rights and safety of consumers at all times. Evaluates the quality of nursing practice and participates in quality improvement initiatives to improve consumer outcomes. Ensures documentation meets service standards and legal requirements. Successful applicants must be registered with the Nursing & Midwifery Board of Australia (AHPRA) as a Registered Nurse and have relevant post-registration experience in an acute mental health environment. You will have experience well-developed written and verbal communication & interpersonal skills and ability to work collaboratively in a multi-disciplinary environment. Successful applicants will have basic computer skills & knowledge and a current national police clearance.
