About The Role As a Clinical Care Partner, you will work closely with clients, families and the wider care team to deliver high-quality clinical support across a range of home care programs. No two days are the same, with responsibilities including clinical assessments, care planning, nursing interventions and supporting clients to achieve their goals. This role offers the perfect balance of autonomy, meaningful client relationships and a supportive team environment. Key Responsibilities Conduct comprehensive clinical assessments and reviewsDevelop and maintain person-centred care plansDeliver nursing interventions within your scope of practiceSupport clients with wound care, medication management, chronic disease management, diabetes care and other clinical needsMonitor client wellbeing and identify clinical risksCoordinate referrals to GPs, allied health and specialist servicesProvide clinical guidance and support to Care Partners and Support WorkersMaintain accurate documentation and compliance with funding requirements About You To Be Successful In This Role, You Will Have Current AHPRA registration as a Registered NurseCurrent First Aid and CPR certificationCurrent driver's licence and access to a reliable insured vehicleNDIS Worker Screening Check (or willingness to obtain)At least 1 year of experience in community nursing, home care or a similar clinical settingStrong assessment, clinical decision-making and communication skillsA genuine passion for person-centred care Why Join Apollo Care?
