We are currently seeking Support Workers across the Sydney Inner West region.

About the Role As a Support Worker, you will play an important role in enhancing the wellbeing and quality of life of our clients by providing person-centred care and support tailored to their individual needs.

Duties and Responsibilities Personal care assistance, including showering, grooming, dressing, and toileting Domestic assistance, including cleaning, laundry, meal preparation, and shopping

Social support and community access Transport assistance to appointments and community activities Medication prompting (where applicable)

Monitoring and reporting changes in clients' wellbeing Maintaining accurate documentation and progress notes Providing care in accordance with client care plans, policies, and procedures
