As an innovative rural health service, Hesse provides acute, aged and community-based services across parts of the municipalities of Colac Otway, Golden Plains and the Surf Coast.
About the Role
Hesse is committed to leading health and wellbeing by providing best practice health, aged care and community services to enable the wellbeing of our community. The role of the Community Health Care Worker is to support recipients of Home Care Services to maintain their independence by directly assisting them with a range personal care, domestic and community living tasks in accordance with agreed identified needs and services as documented within the care plan. The community care worker will undertake their work in the participants home or community setting. Shifts will be based from either the Winchelsea or Bannockburn Hesse sites. The position is supervised by the In Home Support Team Leader and operates within the Community Services program area.
