This rewarding role involves working closely with individuals, children and families to provide practical support, advocacy and access to services that promote wellbeing, independence and positive outcomes.

The successful candidate will have strong interpersonal skills, experience working with diverse communities, and a commitment to supporting vulnerable individuals and families.

Key Responsibilities

  • Assess the needs of individuals and families and develop appropriate support plans, programs and interventions.
  • Conduct client interviews and consultations to identify challenges, risks and support requirements.
  • Deliver support services and educational programs aimed at improving family wellbeing, resilience and community participation.
  • Monitor client progress, maintain accurate case notes and prepare reports on outcomes and ongoing support needs.
  • Refer clients to specialist services and external agencies where additional assistance is required.
  • Identify community needs and available resources relating to housing, health, welfare, employment, education and training opportunities.
  • Liaise with community organisations, government agencies, service providers and local businesses to improve access to support services and community resources.
  • Advocate for clients and promote awareness of programs and services available within the community.
  • Provide practical assistance, guidance and support to families, children, young people, people with disability and other individuals requiring support.
  • Assist young people and families to address social, emotional, behavioural and financial challenges.
  • Contribute to the preparation of reports, funding submissions and documentation required by government departments and funding bodies.
  • Participate in case reviews, team meetings and continuous improvement activities to support quality service delivery.

Skills and Experience

  • Relevant qualification in Community Services, Welfare Work, Social Work, Human Services or a related field.
  • Demonstrated experience working with individuals, children and families in a support or community services environment.
  • Strong communication, case management and relationship-building skills.
  • Ability to work effectively with people from diverse cultural and social backgrounds.
  • Sound report writing, record keeping and administrative skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Provide practical assistance, guidance and support to families, children, young people, people with disability and other individuals requiring support.
  • Assist young people and families to address social, emotional, behavioural and financial challenges.
