The Sunflower Hub team is growing, and we’re hiring experienced, reliable Support Workers to deliver services to NDIS participants in their homes and communities.

In this role, you’ll help people live their best lives while growing your own skills, within a team that values your development and wellbeing.

About the role

From specialist disability accommodation to in-home support and community access, we make sure participants feel seen, heard and safe.

You’ll provide high-quality, person-centered support to help participants with daily living, build life skills and work towards their NDIS goals.

Your responsibilities will include:

  • Delivering personalised support in line with individual needs

  • Following health care plans and protocols to ensure consistency and quality of care

  • Administering medication, providing personal care, and assisting with health-related needs

  • Supporting participants to engage and participate in their community

  • Writing quality shift notes and maintaining accurate records, care plans, incident reports and behaviour observations

  • Supporting individuals to manage challenging behaviours by following behaviour support strategies

  • Encouraging independence by creating opportunities to learn and practice new skills

  • Working closely with participants who require complex physical and behavioural support. Experience with hoist use and manual handling is essential

- SCHADS award aligned pay with great penalty rates:

  • Weekdays: $36.23/hr

  • Saturdays: $50.72/hr

  • Sundays: $65.21/hr

  • Public Holidays: $79.70/hr

  • Australian Driver’s License
