Nurse Practitioner / Transitional Nurse Practitioner – Palliative Aged Care Supportive Service (PACSS)
Make a meaningful difference in the delivery of person-centred palliative care for older people.
An exciting opportunity exists for a highly skilled Nurse Practitioner (NP) or Transitional Nurse Practitioner (TNP) with advanced clinical expertise in Palliative Care and a strong interest in Aged Care to join the Palliative Aged Care Supportive Service (PACSS).
In this role, you will deliver advanced nursing assessment, diagnosis, and clinical care to patients and their families within a defined scope of practice, consistent with the NSW Health Nurse Practitioner Framework and relevant professional standards. You will also provide clinical leadership, contribute to strategic service development, and support governance processes to strengthen models of care for residents transitioning to end-of-life care.
About the RoleAs the NP/TNP within PACSS, you will provide expert palliative care to older people residing in Residential Aged Care Facilities (RACFs) across the Ryde and Royal North Shore catchment.
Working collaboratively with the NSLHD Specialist Palliative Care Service, the Aged Care Rapid Response Team (ARRT), General Practitioners, and HammondCare Palliative Care, you will:
